This means that using a network dentist will provide you with a higher level of coverage and may help lower your dental care costs.

Get 2 virtual dentist visits1 in addition to the routine in-person exams included in this dental plan. You may need to pay an out-of-pocket cost for additional visits. Use virtual dental visits to:

The Bronze, Silver and Gold (PPO) plans are available to residents of all states.
The Platinum (DHMO/Exclusive Network) plan is available if your employer offers it and if you are a resident of AZ, CA, CO, DC, FL, GA, IL, IN, MD, MI, MO, NC, NJ, NV, NY, OH, OR, PA, TN, TX, UT, VA, or WI. Please check your employer’s enrollment site to see if the Platinum plan is available.
Bronze, Silver, Gold (PPO)
Yes, preventive care — such as routine cleanings, fluoride treatments and bitewing X-rays — are covered.
There is coverage for fillings, oral surgery and root canals on all plans. Under the Silver and Gold plans, there is coverage for major services such as crowns and braces. For more details, call us at 888-571-5218.
With the Gold plan, there is orthodontia coverage for both adults and children. The Silver plan provides coverage for children under age 18.
There is no orthodontia coverage under the Bronze plan.
Yes, adults (age 18 and older) can get oral cancer screenings as part of your preventive care benefit.
Yes, the plan provides coverage for extra dental visits during pregnancy and the first 3 months after delivery.

Oral cancer screenings are not a covered benefit under the Platinum (DHMO) plan for residents of DC, MD, PA or VA. If you are not a resident of one of these states, oral cancer screenings for adults (age 18 and older) are part of the plan’s preventive care benefit.
