Take your Health Assessment at myuhc.com. On myuhc.com, download the Physican Results Form for your biometric screening by navigating to the Rally Health Survey.

Get Rewarded on Your Journey to Better Health

After you have completed your biometric screening, visit LaCountyGetsFit.com and create a user account.

Once you've registered and created an account, you'll receive a $100 reward certificate for completing your bio via email with a personalized link that you can redeem for a gift card.
